Whether your responsibilities change, you need to be away from work, or your account activity shifts over time, it’s important to review the status of your One Card status and request updates when needed. Staying proactive helps prevent disruptions and ensures smooth business operations.

One Cards expire every four years on the last day of the month shown on the front of the card. If you have not used your card in the 12 months before it expires, the bank will not automatically reissue it.
If your card remains active, the bank will mail a replacement card to your work address on file. You should receive your new card during the month before it expires.

Report a lost, stolen or misplaced One Card immediately to U.S. Bank and Shared Services to protect University funds. You may be responsible for charges made before you report the card missing.
U.S. Bank may detect suspicious activity before you notice it and will contact you to verify recent transactions. Your card will be temporarily blocked until you confirm the activity. You must work directly with the bank to resolve fraud concerns and confirm or dispute any questionable charges.
How to Report Fraud or Compromised Card
If you believe your card has been compromised or you see unauthorized charges, take the following steps right away:
- Contact U.S. Bank immediately: Call the number on the back of your card or the appropriate number listed on this page. The bank will ask you to verify your identity. Be prepared to provide:
- Card number
- Name on the account
- Mailing address listed on the account
- Phone number
- Last four digits of your SSN (use your University employee ID)
- Monthly credit limit
- Destroy the compromised card: If you still have the physical card, cut it up or shred it to prevent further use.
- Review recent transactions carefully: Check your account for unauthorized charges, including small amounts. Fraudsters often test cards with minor purchases before making larger ones. Respond promptly to any requests to confirm or dispute charges.
- Update recurring payments: If you used your One Card for automatic payments, log in to those merchant accounts and update them with your new card information to avoid missed payments.

Most account updates are completed using the One Card Change Form in PeopleSoft Financials, including:
- Canceling a card
- Transferring or terminating a cardholder
- Requesting a spending limit increase
- Suspending a card while on temporary leave
- Updating personal information
- Responding to the annual utilization report

If a cardholder needs to update the name, address or phone number on file, the cardholder should complete the One Card Change Form and indicate the appropriate changes.
Cancel or Change Ownership
Departments must take timely action when employment status, job responsibilities or contact information changes. Prompt updates to your One Card details help protect University funds and ensure appropriate oversight.
Cardholder Transfers or Terminates
If a cardholder leaves the University or their employment status changes such that they no longer are an appropriate cardholder for the department, the department must cancel the One Card account by submitting a One Card Change Form and destroy the physical card within 48 hours.
Student One Card Custodians
Similarly, if the custodian for a student One Card experiences a change in employment status or responsibilities, the department must act within 48 hours to notify the FIN Structure Responsible Individual and submit a Custodian Change Form to assign a new custodian.
Cardholder on Temporary Leave
If a cardholder goes on temporary leave, there are two options:
- Temporarily suspend the account by submitting a One Card Change Form.
- Leave account active while on leave to allow pending or recurring charges to post.
The cardholder must not share the card with anyone else to make purchases on their behalf while on leave.
